“My role is to navigate patients through our system, coordinate their
care needs and help all of the clinicians shine,”Weaver said. “This
involves everything from managing patient flow to doing research up
front to make sure that patients have the referrals they need and that
any obstacles to care – such as not having transportation –
are removed.”
The Center sees 40 to 50 pediatric patients a day, and each visit is followed up with a
care plan so patients don’t end up in the hospital or in an emergency room unnecessarily.
“In obstetrics, it’s critical that the high-risk patients don’t fall through
the cracks,”Weaver said. “Nurses follow up to make sure their
appointments are scheduled, that there aren’t any knowledge deficits
and that the patients follow their plan of care.”
For Guevara, The Center has made an enormous difference in the way she views her
pregnancy and is giving her the confidence to start her new journey as a mother.
“ I’m not worried about anything or nervous,”
Guevara
said.
“It feels natural to be here, and I know I’m doing
the right thing for my baby.”
